Sometimes the topnut works loose enough for the adjusting nut to come loose and then everything is loose. What usually happens though is that the bearings in the cages wear into the cups and you need a new headset. They come in various sizes and types so you will either have to take the bike in or take all the parts from your headset in to get a replacement. You should try backing the topnut off some more and then tightening the adjusting nut down as much as you need to remove the play and still not have any friction while your steering and then tighten the topnut back down. It will require a thin walled bicycle wrench the correct size to hold the adjusting nut in position so that you don't tighten it further while you are tightening the topnut with a second suitably sized wrench.
